A critical review of the evidence behind blood glucose monitoring in patients with type 2 diabetes mellitus.
The skill chosen is blood glucose monitoring in those with non insulin dependent diabetes mellitus (type 2 diabetes). The skill is used in all types of clinical settings but it was used in a community psychiatric setting where residents with a diagnosis of non insulin dependent diabetes mellitus required daily blood glucose monitoring. The World Health Organisation (2013), define diabetes as “a chronic disease that occurs when the pancreas does not produce enough insulin, or when the body cannot effectively use the insulin it produces”. Type 2 diabetes occurs when the insulin is present but not active. Type 2 diabetes is often linked with the use of atypical neuroleptics such as clozapine used to treat schizophrenia (Sernyak et al (2002). Some of the residents in the particular psychiatric setting were prescribed a high dose of clozapine and had developed type 2 diabetes therefore the nurses carried out blood glucose monitoring daily. Kirk and Stegner (2010), recognise blood glucose monitoring as an essential part of the management of diabetes mellitus and is one of the most important skills in achieving a good quality of life for these patients. Although the majority of patients are capable of carrying out this skill, Renda (2006), acknowledges that it is a skill which demands a degree of dexterity and proper judgment to adequately carry out the test and to make sense of the results. It is patients who do not possess these attributes that require the nurse to undertake the skill for them, and he or she must be competent in doing so. However the debate continues whether diabetes management has become too focused on monitoring blood glucose levels. The reasons both for and against self blood glucose testing in non insulin dependent diabetes and the technique for undertaking the skill shall be discussed in this assignment.
