(Reason for referral):
Jose was referred by the LEP Committee/ Child Study Team for Language Proficiency/Dominance Assessment in result of academic difficulties.
Current level of performance:
Jose is currently repeating the 1st grade for the second time. He scored a four on the initial M-DOLPS-R and is entering the English for Speakers of Other Languages (ESOL) program on August 27, 2003. He has participated in the ESOL program for four semesters, for two hours daily. Jose also receives Home Language Arts (Spanish-S) instruction for one hundred fifty minutes a week.
Jose was give the International Performance Scale to assess his intellectual functioning
The results of Jose’s performance profile reflect the following findings:
M-DCOLPS-R: ESOL level 3, Social Language Inventory: Very limited-Independent,
Spanish: High Immediate, English: Low Intermediate.
Jose was also administered the Adaptive Behavior, Social History, Psycho-Educational, Vision, Hearing, Speech/Language, and Observation exams where he scored lower than normal. His IQ is 108 which places him the in the average range of intellectual functioning.
Goals & Benchmarks:
Within the year Jose will be able to have a grasp on recognizing words and letters by sight with 80% effectiveness.
-By April 2012 Jose will be able to recognize small words like at, and hat.
-By June 2012 Jose will be able to recognize medium size words like drive and dive.
-By August 2012 Jose will be able to recognize both small and medium size words like dog, cat, house, and bird by sight with at least 50% accuracy.
-By December 2012 Jose will be able to have a grasp on small and medium words and will be able to start on hard words such as dictionary and follow.
Jose will be tested weekly with teacher made test.
Within the year Jose will be able to elaborate on his feelings and be able to express it verbally.
